Optional Long-Term Services


All states offer some optional services.

Institutional Care

States have the option of covering the following services:

  • Inpatient hospital and nursing facility services for individuals who are 65 or older and live in an institution for mental diseases (IMD).
  • Services in an intermediate care facility (ICF) for individuals with mental retardation (ICF/MR).
  • Inpatient psychiatric hospital services for individuals under age 21.

Non-Institutional Care (Home- and Community-Based Services)

States can offer the following home- and community-based services:

  • Home health services to individuals who are in optional eligibility categories. (The same services must be offered as are available to those who are in the mandatory eligibility categories.)
  • Respiratory care services for ventilator-dependent individuals
  • Case management services
  • Personal care services
  • Private duty nursing
  • Hospice care
  • Adult day care services
  • PACE (Program for All-Inclusive Care for the Elderly) program services 

States have wide latitude through waiver programs to cover other home- and community- based services that will help people avoid institutionalization.
